I have some more examples:
The Ringer (2005) Widescreen & Open matte/Pan & scan Fullscreen on two sides of its 2006 DVD
The Secret of NIMH (1982) Widescreen & Full Screen open matte (other than five cropped shots) on Disc 1 of the Family Fun Edition DVD
