You are right, it was only lining up with the subtitles in the editor (no live video preview). I am doing a re-render of the video with sporvs settings, then combining it all back together to see how it looks. I will check that audio delay as well.
OK finally making some progress now! The new video render and audio came out the same as the original, so then I started looking into the audio delay. I used a VHS rip to compare to and saw that this one was -100ms off from it so I decided to roll with that. It seems to have pretty solid sync now.
The subtitles are the last thing, and they are still a work in progress. Looks like they stay nicely in sync until about half way then go a few seconds off. I'll need to track down which line it starts on and correct that. Hopefully that should be it.

There's a problem with the PGS subtitles, so I'd recommend using the ASS for now if you can. Some lines that are back to back come out delayed. If I recall correctly, avs2bdnxml doesn't like it when one line ends on the same exact timecode the next begins with. I think the only way to fix it is to manually go through the timings and push them apart by a few ms.
